What is the difference between 4T and youth XS?
Short answer
They overlap, and which one is bigger depends on the brand. Measured flat, a 4T is 14 inches across the chest and 17½ inches long in both toddler blanks we checked. Youth XS ranges from 13½ to 16 inches across and 18 to 20½ inches long. So youth XS is always the longer shirt — but a Hanes youth XS is narrower than a 4T, while a Gildan Ultra Cotton youth XS is two inches wider, which is four inches the whole way round.
The T in 4T stands for toddler, and toddler patterns are drafted around a body shape that is genuinely different from an older child’s: shorter torso, rounder middle, proportionally larger head. Manufacturers allow for nappies at the hip and widen the neck so the garment goes on without a struggle.
Youth XS assumes none of that. It is a scaled-down version of the older children’s pattern — narrower at the hip, longer in the body, with a standard neck.
Length is the part that behaves predictably. Every youth XS in the tables below is longer than a 4T, by half an inch to three inches. Width is the part that does not: youth XS is the first size of a range, and where each manufacturer starts that range is a decision rather than a standard. That is why one parent insists youth XS is far bigger than 4T and another insists they are identical. Both are describing the shirt in front of them accurately.
The practical consequence is that the same child can measure into both and wear only one comfortably. If the shirt rides up at the front, the toddler cut was too short. If it hangs loose at the hip and the neck gapes, the toddler cut was too generous and youth XS is the better choice.

What 4T and youth XS actually measure
Chest is measured flat across the shirt, so double it to get the way round. Body length runs from the high point of the shoulder to the hem.
4T
The two toddler blanks agree exactly, which is why 4T is the more predictable size to buy unseen.

Youth XS
Bella+Canvas does not make a youth XS at all — its youth range starts at S (6–8).

Across these blanks, the chest ranges from 13½ to 16 in across — 2½ in of difference, or 5 in the whole way round, and the body runs 18 to 20½ in long.

Manufacturers allow a tolerance on every measurement, so an individual shirt can differ by up to an inch. We have not measured these shirts ourselves.

Would a youth XS fit a 4T?
Usually, yes, and in a Gildan it will be roomier rather than tighter. A Gildan Ultra Cotton youth XS is 16 in across the chest against a 4T’s 14 in. A Hanes youth XS is 13½ in, so that one runs slightly narrower than the 4T — but it is still half an inch longer in the body.
What size is youth XS equal to?
Youth XS is normally labelled 4 or 4/5 and is the first size of the youth range, directly above 4T and below youth S (6/7). Measured flat it is 13½ to 16 in across the chest and 18 to 20½ in long, depending on the manufacturer.
Is youth XS the same as 5T?
No. 5T is still a toddler pattern — wider at the hip and shorter in the body. A 5T measures 15 in across the chest and 18½ in long, which makes it wider than a Hanes or Gildan Softstyle youth XS and shorter than a Gildan Ultra Cotton one.
What is the difference between size 4 and 4T?
A plain 4 is the youth size — it is what Gildan and Next Level print on their youth XS. It is cut slimmer at the hip and longer in the body than 4T, which is the toddler version for the same age.
